It's not so uncommon to see celebrities ruin their appearance by getting plastic surgeries. In the previous century this wasn't as big of a trend as it is now. Michael Jackson was probably the first celebrity of his popularity that absolutely ruined his appearance to no return. Joe was observing this progression at the time and it was crazy to watch.
