The iris is a wimpy tissue that is very delicate and susceptible to damage during cataract surgery. This is relatively common and it is safe to say that 100% of cataract surgeons will experience a case where there is iatrogenic iris damage. This case shows an area of iridodialysis where the iris root has become detached, creating an additional pupil and causing the patient monocular diplopia and glare.
