The idea behind the double capsulorhexis is to avoid the capsule run-out (Argentinian Flag Sign) for intumescent white cataracts that are pressurized with liquefied lens cortex. In this video, a resident surgeon first does needle decompression to remove some of the intumescent fluid to de-pressurize the capsular bag. Then a small initial capsulorhexis is created since it will be strong without weak areas. Next the nucleus is spun and tilted to ensure that all of the intumescent fluid has been removed, including from behind the nucleus. This is a very useful technique that will decrease your stress as a surgeon and give the patient a better outcome.
