Captains surviving decapitation were a big point of discussion when the game came out. Reports say people at Monolith weren't able to recreate this. I've managed to do it over 5 times with a sequence of steps. All of which I sent detailed information to Monolith so they can fix it for future projects. Also, I'm not revealing how to do it.
