1st week Immediately following the surgery:
Control pain with prescribed medication
Rest and relax and allow time for proper healing
Limit physical activity and Effort
No lifting objects greater than 5-10lbs
Prevent Constipation (see video link below)
2nd to 3rd week
Light walking
Pelvic Bracing (abdominal muscle activation)
Bladder urgency and frequency control
Pelvic Floor muscle activation (Kegels)
Pelvic Floor Physical Therapy (assist in increasing control and improving quality of life sooner)
4th week
Increase activity level
Gentle resistance training
Pelvic PT - Electrical stimulation if voluntary control of pelvic floor muscles is not present
Follow the doctor Protocol on ED medication
5-8th week
Continue increasing exercise level
Increase cardio - help with circulatory system and increase blood flow everywhere including the genital
Follow the doctor Protocol on Ed medication and Vacuum
WHY IS IMPORTANT DO DO PELVIC FLOOR PHYSICAL THERAPY
Because 50-60% of male post-prostatectomy will experience loss of bladder control and decline in sexual function (erectile dysfunction) due to the physiological and anatomical changes that take place after surgery.
Also over 50% of men ages 40-70 experience a decline in sexual function due to erectile dysfunction which has been correlated with the following comorbidities: high BMI, high blood pressure, diabetes, sedentary lifestyle, poor nutrition, stress, medications, enlarged prostate, radiation, and chemotherapy.
Research shows that improving the cardiovascular system helps to increase blood circulation in the body, which decreases blood pressure, manages diabetes and when combined with Pelvic Floor Muscles exercises can improve erection based on IIEF-EF (erection questionnaire)as much as 62%.(Chen et.al.)
Also pelvic floor muscle electrical stimulation can help decrease muscle atrophy, increase blood circulation, prevent loss of penile muscle tone (maintain girth and length). Research shows that scores on IIEF-EF (erection questionnaire) improved as much as 47%( By Van Kampen et. Al.
OVERALL MESSAGE - Receiving an individualized program from a trained Pelvic Floor Physical Therapist can help in decreasing the effects of erectile dysfunction in patient post-prostatectomy as well as in healthy individuals with vascular origin erectile dysfunction.
