This video teaches the surgical steps involved in limb sparing surgery for removing (resecting) a bone sarcoma called an osteosarcoma arising from the left proximal humerus “upper part of arm near shoulder.” This is an extra-articular resection that includes the proximal humerus and glenoid (ball and cup). Sarcomas of the proximal humerus that extend outside the bone cross the shoulder joint easily and contaminate the glenoid so in many instances the glenoid is removed with the humerus to minimize the risk of the osteosarcoma recurring (coming back in the shoulder girdle region). An intra-articular resection would include the proximal humerus only. The shoulder is reconstructed in a way to provide stability and restore function of the limb and shoulder using a special tumor prosthesis. The prosthesis can be sized and assembled in the operating room. Special steps are taken to stabilize the prosthesis with static suspension methods using heavy non-absorbable sutures and dynamic suspension methods with multiple muscle flaps. The muscle flaps also cover the entire prosthesis with good soft tissue and protect it from infection as well as restore some external rotation. They ultimately scar around the prosthesis and stabilize it. This is a limb sparing surgery as opposed to performing a forequarter amputation of the arm. The procedure described in this video is a reliable method for restoring stability of the shoulder and function of the hand and elbow. Specific protocols for physical therapy and use of a sling or a brace for a period of time must be adhered to postoperatively for an optimal result.
