New evidence into the fatal Taipan helicopter crash off Queensland last year suggests a helmet device could be blamed for the incident.

The device shows the pitch and roll of the aircraft.

It has been deemed as an ‘unacceptable risk to flight safety’ by experts.

Some experts have added that they warned defence of the ‘substantial risk’ of multiple deaths.

They believe that the display was ‘ambiguous’.
