There is so much to be learned from this case. The patient has advanced diabetic retinopathy and a significant cataract. When the phaco probe was introduced into the eye, it became evident that there was no posterior capsular support. A few weeks prior, the patient had an intra-vitreal injection that damaged the posterior capsule and the patient was then referred to our guest surgeon Dr Utku Limon from Istanbul, who performed a combined cataract surgery and pars plana vitrectomy. The patient has fibrovascular changes from the proliferative diabetic retinopathy and the vitrectomy was needed to restore retinal anatomy. Dr Limon does a great job in performing this surgery and we can learn so much from his skill.
