Pre-Season 4 Update:
-Start Boots/3pots against painful skillshot dependent champions (ahri, lux, etc) or against strong ganking junglers (j4, naut, etc)
-Start cloth/5pots against AD mids (talon, zed, etc)
-Start Dorans/2pots against everyone else.
-You want to rush tear of the goddess in most cases, as Seraph's embrace is just too strong right now. Tear is super easy to stack with Cassio (especially when you have blue), and the huge mana boost allows for Cassio to participate in extended teamfights without running out of mana. Also, since Cassio doesn't have a great escape, the active is even more useful.
-Get T1 boots and WotA after tear in most cases. Cassio's sustained damage (especially her single target damage with her E), effectively doubles her health in long teamfights and in early 1 v 1 trades with the help of WotA's spell vamp
-After this rush sorc boots and then Rylai's for the slow.
-Void staff is your last core item, but it shouldn't be bought until you see the enemy team start stacking magic resist.
-Buy Rabadons early if you are way ahead to keep your lead, otherwise leave it for final item in situations where you have nothing more imperative to buy.
-Buy Haunting Guise after Tear and eventually upgrade it to Liandry's against a high health team (udyr jungle and mundo top, for example).
-Buy seeker's armguard and eventually upgrade it to Zhonya's against mostly AD or dive comp team.
-Buy Chalice instead of tear when against 2-3 AP champs on enemy team, and when you need early magic resist (I'm looking at you LeBlanc).
-Replace WotA with Abyssal scepter when your team AND the enemy team have 2-3 AP champs each.
