This is the consent I currently use and document for telemedicine:

"This encounter was completed via two-way synchronous audio and video communication. Patient's identity was confirmed, and patient's location was confirmed to be in ____. I obtained verbal informed consent from patient to proceed with virtual visit. Patient voiced understanding regarding benefits, risks, alternatives, and option to not participate. Patient desired to proceed with telemedicine visit."
