This is the Ganro (formerly known as Golden Sun) TR-322 tilt rotary table sold by CNC Indexing in the United States. This rotary table can be tied into a machine control for full 4th/5th axis use or it can be setup with one or both axes using a stand alone index control. This particular model has a 12.5" faceplate. Available sizes in the tilt table design are 6", 8", 10", 12.5", 16", 20", 25" and 31.5".
