Result of a failed active lifter. This is said to be just a bad batch where the disconnect pin inside the lifter breaks. Reports suggest that most get replaced and do not come back. Here I'll be replacing all 16 active lifters with new GM lifters and the 2 push rods with the associated cylinder. I found the push rod for the exhaust on the affected cylinder was bent.
