Before completing the story I had collected all available "Soul-Melody's" and "Cosmetic Items" available on the map however these two trophies appear after finishing the story.

Trophy: Thing
Description: Find and Soul-Jump into Pwi Dua's hand
During the end cutscene you cut off "Pwi Dua's hand" when you're at "Ga Ngaza". Once you have control of Tchia go back "Ga Ngaza" and Soul-Jump to it to collect this trophy.

Trophy: Ça de Wizz!
Description: Reach 100% Game Completion
To obtain 100% Game Completion you will need to complete the story and visit the two gravestones post game as well as complete everything in the Journal section. All collectables, campfires, docks, activities etc. are on the map and visiting those areas to complete them should also unlock all the Landmarks and Flora.
What you may be missing is Fauna (Fish) as they are not on the map and will require you to visit specific locations to find them. About 50% of the fauna can be found in "Ieji Sinoe" so visit there and soul-jump into everything. After doing this I had 2 remaining fish which was "Duong" and "Trevally" which can be found here:
15:08 - Duong located around "Dopwa"
17:35 - Trevally located around "Oopa"
21:08 - Visit two gravestones after completing the story
Once all of this is done you should obtain this trophy.
