The key to proper measurement is to measure the size of the duct and not the overall size of your existing vent cover. This requires removal of the currently installed vent cover.

Floor vent covers typically can just be lifted out. Wall vent covers and ceiling vent covers often require unscrewing to remove your current vent cover and expose the duct for proper measurement.
The most common duct sizes used by builders today are 4" x 10", 4" x 12", 6" x 10", and 6" x 12" but there are applications where custom or non-standard sizes are needed.
Another key point of consideration is the depth of the damper. The damper closes off the airflow and is attached to the face of the vent cover. This requirement is 2.5" - 2.75".
