Delivery and Payment Reform Technical Assistance Call #1: Primary Care and Public Health: Linking Public Health and Advanced Primary Care to Improve Outcomes (Recorded Jan. 9, 2015)

ASTHO is committed to supporting state and territorial health officials and their leadership team's work in payment and delivery reforms. In the summer of 2014, ASTHO surveyed state and territorial health agencies to assess their needs and current activities in regard to payment and delivery reform initiatives and, when applicable, State Innovation Models (SIM) grants. This four-part technical assistance call series will focus on some of the key areas that states identified as needing additional training and technical assistance. This call is the first in the series and will be focused on developing opportunities for partnership between public health and various models of advanced primary care.
