I want to discuss some of my preoperative pet peeves, which sometimes patients do, which can lead to their surgery to be canceled or to a poor outcome, or can be even dangerous to their health. The first one regards smoking. We ask all our patients to not use any nicotine for at least 30 days before surgery. Sometimes patients come in and say, "Well, doc, I only had one cigarette." One cigarette is enough. If your urine nicotine test is positive, your surgery will be canceled.

Often patients use a supplement in order to lose weight and meet their BMI goals. One of the most common ones that is used is called phentermine. The problem with phentermine is that it's very close to amphetamines and these types of supplements can affect you during your general anesthesia and can lead to very dangerous things, such as a heart attack. Therefore, we must be sure to be off phentermine or any related type supplement for at least 30 days before surgery.

The next pet peeve has to do with oral birth control and any estrogen related supplements prior to surgery. All of these have to be stopped at least 30 days before surgery, because they do increase your chance of blood clots, which can start in the legs and then go to the lungs and cause serious problems. So we do ask all our patients to stop them, but then of course you have to be careful because one of the things that we are going to test as well, is for pregnancy. A few times we get patients that were off their birth control, but then come in and their pregnancy test is positive. So be very, very careful with that, as well.

My biggest pet peeve regarding your preoperative evaluation is non-disclosure or emission of information. Please make sure to detail your entire medical and surgical history. If we're not aware of something, that can lead to potentially very dangerous outcomes, such as blood clots or heart attacks, okay. Here at Pure, safety is our number one concern.
