Case & Plan :
An 89-year-old woman presented with posterior and lateral wall ST-segment elevation myocardial infarction on September 10, 2010. Cardiac catheterization revealed 3-vessel coronary artery disease with left main bifurcation disease and left ventricular ejection fraction of 40%; (SYNTAX score 38). Patient underwent primary percutaneous coronary intervention (PCI) of circumflex obtuse marginal with intra-aortic balloon pump support and did well subsequently. Staged PCI of left main bifurcation and left anterior descending artery is planned.
