AWVs are different for annual Physical Exams. Annual Wellness visits are covered by Medicare and is NOT considered to be a “head to toe” exam.
Within 12 months of Medicare Part B enrollment, the patient is supposed to have a “Welcome to Medicare visit” also known as an IPPE (Initial Preventative Physical Exam) and then once every year there will be a subsequent AWV.
If the patient is at the AWV and is complaining of other health issues and those issues require separate documentation and “work”, then a problem oriented E/M code and modifier 25 must be appended to the claim so the provider can be adequately reimbursed.
