Procedure : Endoscopic Variceal Ligation (EVL) at SQUARE HOSPITAL LTD Dhaka Bangladesh.

FINDINGS:

OESOPHAGUS: Multiple columns of grade IV oesophageal varices are seen at the lower half. First session of band ligation is done by 7 applications of Boston Scientific band ligator. Gastro-oesophageal junction is located at 40cm from incisor teeth. No evidence of hiatus hernia present

STOMACH: Fundus, body & antrum examined. No food residue, foreign body or blood is seen. Mosaic pattern mucosal lesion is seen in the body and fundus.

DUODENUM: First (D1) & second (D2) part examined. No blood in the lumen. There is no suggestion of ulcer, erosion, diverticulum or duodenal scarring. No nodularity. tumor or polyp is seen. Mucosal folds in D2 are normal at endoscopy.

BIOPSY: Not taken.

COMMENTS: Grade IV Oesophageal Varices with Congestive Gastropathy

THERAPEUTIC INTERVENTION: First session of Endoscopic Variceal Ligation (EVL) done.

ADVICE: Review Endoscopy and possible EVL/ Sclerotherapy after 1 month.
