After the events in the previous film, Sam has broken up with Agung and he is currently in a dilemma in fighting for his true self from his increasingly strong dark side despite the presence of the kuntilanak's divine revelation. This true self desires to kill as many people as possible by summoning the kuntilanak. Sam also often dreams about people who were killed by the kuntilanak, such as his best friend, Dinda, and Mangkoedjiwo's great-granddaughter, Sri Sukma, who regrets using the kuntilanak. Currently, Sam lives in a boarding house of a Chinese merchant family. The daughter of the family, Yenny, is immediately afraid when she meets Sam, but her parents do not know the reason. On the other hand, the remaining followers of the Mangkoedjiwo sect, who have just lost Sri Sukma, are facing many problems because they can no longer control the kuntilanak. They plan to force Sam who has divine revelation to join their sect to continue their evil plans by tracking down Yanti, Sri Sukma's assistant and Sam's former landlady who has been hiding since Sri Sukma was killed. Meanwhile, Agung, becomes depressed because he was once kidnapped by a kuntilanak. Agung is then persuaded by his best friend, Iwank, to meet Sam again and destroy his problem from the root through his investigation of the Mangkoedjiwo sect. One night, Sam calls Iwank and sings durmo until Iwank dies in an accident while cooking. This motivates Agung to look for Sam. On another night, Sam faces his dark side that wants to take over Sam's body, but Sam manages to defeat it. After a long search, Agung manages to track down and meet Sam who has just returned from visiting the grave of his mother, Mega N. Widjoko. Sam initially did not want Agung to interfere in his affairs, but after being persuaded and thinking, Sam allows it and the two of them make peace. Through the results of Iwank's investigation, Sam and Agung find the writing "Oejong Sedo" which is the name of the place where the kuntilanak's wangsit originated. Sam and Agung meet Yanti, who had previously contacted Sam through a vision so that he would visit her. Yanti informs that the Mangkoedjiwo sect has been looking for Sam to join their sect and that the kuntilanak is increasingly free under Sam's control and will eventually turn on him since Sam destroyed the mirrors to summon the kuntilanak. When Sam asks how to stop the problem, Yanti asks when Sam last visited his mother's grave. A moment later, three followers of the Mangkoedjiwo sect come and kill Yanti, but Sam sings a durmo to summon the kuntilanak which kills two of them.
Sam and Agung hide in Sam's boarding house. They are helped by Yenny's grandfather, but the grandfather is revealed as one of the followers of the Mangkoedjiwo sect. After killing Yenny's parents, the Mangkoedjiwo sect captures Sam, Agung and Yenny, then takes the three of them to the sect's meeting place. There, Sam is forced to vent his anger so that the followers of the sect can teach Sam how to control the summoning. Agung is badly injured when attacked by one of the followers of the sect, but he is able to convince Sam in his subconscious to find his true identity before Agung dies. Making his choice, Sam orders the kuntilanak to kill all the followers of the Mangkoedjiwo sect while he escapes with Yenny, but in the courtyard of the building, Yenny is kidnapped by the kuntilanak, leaving Sam confused to suddenly meet his mother.
