Form J: Return Of Reportable Accident

Form K: Return Of Minor Accident

Form L: Notice to be posted on notice board by owner, agent or manager on or before 20th Feb of every year.

Form M: Notice of initial/ periodical medical examination under rule 29B/ 20 days before exam

From N: Second and final notice of initial/periodical medical examination under rule 29 B/ within 10 days of fail to appear medical exam

From O: Report of medical examination under rule 29B / Medical Report

From P: Medical Standard of Fitness for Persons Employed / Standard of Medical Examination

From P1: Medical standard of fitness for persons to be employed in mines.

From Q: Notice of medical re-examination by Appellate Medical Board ( Within 30 days of request by appellant)

From R: Second and final notice of medical re-examination by Appellate Medical Board

From S: Standard and report of medical re-examination by the Appellate Medical Board

Form T: Annual Return for the year ending on 31st Dec
( on or before 20th February)

From U: Inspecting report by workmen’s inspector / Action will be taken by the manager on the basis of this form U.

Schedules:

1st: Under MMR 1961 ( Form I, Form II, Form III, Form IV-A, Form IV-B, Form IV-C, Form V, Form VI)

2nd: A list of equipment should be contained in First Aid Room.

3rd: A list of equipment should be contained in First Aid Station.

4th: Exemption from hours and limitation of employment

5th : Abstracts of the Act

6th: Observance of Local time
