Thai One-Two-Go airline's flight OG-269 failed to go around at Phuket International Airport in September 2007. The MD-82 aircraft crashed next to the runway and was completely destroyed. The aircraft was carrying 123 passengers and a crew of seven. 90 people died. The pilots made mistakes, but the airline's poor safety culture can be seen as the main culprit in the disaster.
