#GERD ( Gastro esophageal Reflux Disease) #treatment essentially include life style changes and minimum use of medications, and surgery or endoscopy treatment in rare cases.
#Heartburn #Acidity (#Tezab), burning in chest (Chhati mein jalan), acidic burps (#khatti dakar), water bash (Muh mein khatta pani) are the common #symptoms.


GERD is a condition, where stomach acid enters the esophagus. This can occur due to loose valve at lower oesophagus and #hiatus #hernia

Risk factor for the GERD are
Smoking: Smoking increases the GERD symptoms
Obesity: Obesity increases the intraabdominal pressure and increases the GERD

Symptoms of GERD are

• Typical
Heartburn is burning sensation behind the sternum or central chest bone
Acidity or Acid regurgitation is return of acidic contents in mouth
• Atypical symptoms:
Severe chest pain in central part may occur
Acid reaching larynx or wind pipe can lead cough, asthma, sleep disturbance
Acid reaching mouth can lead to dental caries


Diagnosis: GERD can be diagnosed by history of patient symptoms, tests are usually not required.

Investigations
• Upper GI endoscopy: Patients with non response to treatment, prior to surgery, Barrett surveillance
• 24 hour pH study: Patients with non response to medical therapy, prior to surgery
• Esophageal manometry : Prior to surgery if required

Complications:
Esophageal stricture- peptic stricture leading to dysphagia
Barrett esophagus, dysplasia and cancer
Esophageal ulcer causing bleed

Treatment:
• Avoid stress,
• Dinner-bed time gap 3 hours,
• Sleep on left side,
• Sound sleep,
• Raise head end of bed,
• Listen to music,
• Silence in room

• Medications
• Surgery: Fundoplication: Stomach is wrapped around esophagus which increases pressure between stomach and esophagus
• Endoscopic tightening of lower esophageal sphincter
