step by step:
1. Schedule a New Meeting or edit an existing meeting.
2. make sure to select the Required check box of the Registration section.
3. After you saved your meeting, the customize registration options will show up on the bottom.
4. Click edit under the Registration tab.
5. You can custom your own question fields.
6. Check the Required box if you want to make that field required.
