Following the Government announcement to invest £200m into the NHS to boost winter resilience and £40m to bolster social care capacity and improve hospital discharge, Care England is inviting local authority commissioners, hospital discharge teams and integrated care boards (ICBs) to consider an effective discharge solution that will deliver efficient commissioning and discharge from hospitals into the social care sector.
The latest data from NHS England shows, there were over 11,500 patients in hospital who no longer meet the criteria to reside. This number will continue to grow in the coming months as cases of flu and the new Covid-19 variant increase. Whilst pragmatic steps are being taken to roll out vaccines to vulnerable people, the same challenges are faced when attempting to commission social care.
Care England has partnered with Autumna to support local authorities and ICBs to deploy an effective solution for accelerated discharge and care commissioning from hospitals. Autumna’s Dashboard for Accelerated Discharge (D.A.D.) is an automated system meeting the needs of the individual by matching them to the appropriate capabilities and vacancies of local care services by creating a shortlist of available care providers within 60 minutes, significantly reducing the administration burden of commissioning.
Autumna has supported 3,000 families a month to source the care they need locally since January 2023. Over 75% of care enquires find appropriate care through Autumna:
Over 80% are looking to be placed in a care home
Over 10% are looking for homecare services
85% are funding their own care
50% are living with dementia
19% require respite care
Autumna is already increasingly being used by social workers, GPs, and social prescribers as well as members of the public to help them find care and could support local authorities and the NHS to identify available care to discharge patients from hospitals quickly.
Care England is inviting local authority and NHS colleagues to meet with Autumna for an online webinar on 25 September 1300-1400, to learn more about the D.A.D. system and how it can support both local authority and NHS commissioners and discharge teams to overcome the struggle of hospital discharge.
