When most people think of RCA, they envision some type of mechanical or electrical process damage or an injury or fatality. However, the proper application of RCA has nothing to do with the nature of the bad outcome. RCA is a reasoning process to help analyze and understand a gap in performance.

While certainly applicable to the traditional situations described above, RCA can also be applied to administrative situations such as customer service issues, high turnover, employee morale and the like. We can even use RCA to learn why our current RCA efforts might not be meeting expectations!
