Nasopharyngeal angiofibroma occurs mainly in adolescent male patients and is a benign tumor. Its locally aggressive tumor which is very vascular and leads to severe epistaxis almost everyday as it grows into the confines of nasal cavity and Pterygopalatine fossa. The tumor also compresses vital structures like maxillary nerve while growing and derives its blood supply from maxillary artery which usually needs to be ligated to cut off blood supply to the tumor. Here we have Endoscopic Denker's approach used to perform endoscopic medial maxillectomy and remove the tumor piecemeal due to its large size and later drilled the bone to remove residual tumor
