Cataract surgery with a small pupil poses its own challenges. In a patient with pseudoexfoliation, the Malyugin pupil expander is used to dilate the pupil to perform safe cataract surgery. A welcome alternative to iris hooks, the Malyugin ring allows pupillary expansion without additional incisions.
