Some men don’t have children, and they don’t know the cause of it. They learn about its causes over time when tests are conducted and it is discovered as an incidental finding. It can occur without #symptoms and is frequently discovered during an ultrasound. When their female partners' tests are normal and their other tests are normal, #sperm analysis is performed to check the sperm. In this test, sperm quality, quantity, and other parameters like TSH and testosterone hormone levels are checked.
What should be done in this situation?
A lot of #patients come to us with the problem that we have tried our best but are unable to become parents, and they have been diagnosed with varicocele. So, in this video, I will tell you which couples and men can benefit from varicocele treatment. It is difficult to obtain benefits for patients who have varicocele as an incidental finding. Because when these patients go to fertility centers, they are #diagnosed with grade 1 or grade 2 varicocele, and these patients are told to get operated on for varicocele and then they will see what the possibilities are. My suggestion to such patients is that patients with grade 1 or grade 2 varicocele won’t benefit from #varicocelesurgery. This should be very clear because I've received numerous phone calls claiming that I have grade 1 or grade 2 varicocele and can I have surgery? I clearly say ‘no’. Patients with varicocele grades 3, 4, and 5 and sperm analysis showing a low sperm count and poor motility should have varicocele surgery.
The second question is: what is the best treatment?
Most patients avoid surgery and ask for #medicine instead. You need to understand that there are two types of diseases; one type is like #diabetes or high blood pressure, and these can be treated with medicine. Then another type of disease is mechanical damage. Varicocele disease has defects in the valves. In simple terms, valves are like doors that open in one direction. But here, doors are opening in both directions, like in veins, where once the blood is flowing in one direction, these valves are closed to prevent blood backflow. In varicocele, blood is flown back into the scrotum, sperm quality is compromised, and the testes begin to shrink. This cannot be treated by medicine.
So now, if you have to go for surgery only, which surgical #technique is the best?
If you have a good surgeon in your locality, you can opt for microsurgery but before that, make sure to consult a #urologist for the side effects of the procedure. Side effects of the microsurgery are the incision, where veins need to come out and are tied; between these veins, an artery is there. Any damage to this artery can lead to complete damage to that side of the testes. This is the biggest side effect of surgery. The procedure that we perform is called embolization. In this procedure, with the help of an injection, we go inside the vein and work there without touching the artery. There are numerous videos on the internet where urologists explain that embolization is a newer technique that is taught to patients. It is not a fact. Embolization is a well-known, FDA-approved procedure. Now, a lot of advancements have been made in embolization, and due to this, the chances of recurrence, as per my experience, are close to nil. All you need is a good #hospital , the latest equipment, and a good surgeon.
Now the question is what kind of advantage I have with varicocele, #spermcount, or infertility. Studies published till now show that the higher the grade of varicocele, the greater the advantage. So, if you have grade 3, 4, or 5 varicocele and infertility, there are 70-80% chances that your sperm quality will improve.
The next question for patients is: "How many days will it take to improve the sperm count?" Well, it will take 3-6 months. You have to take medicines, then the sperm quality will increase. If varicocele is the only reason for infertility, I would suggest meeting a good urologist, fertility specialist, and #vascular #specialist and talking to them. Discuss with them which #treatment is best for you and proceed accordingly.
