Maternity Allowance, How to Claim and What Do I Need to Send with my Claim Form?
Hello, we are back for the Department for Work and Pensions (or DWP) to continue our discussion on Maternity Allowance.
We have discussed what Maternity Allowance is and who can get it, can you tell me how people apply for it?
Yes, you will need a form called MA1 or Maternity Allowance form. There are a few ways to get this.
You can get a copy online by visiting www.gov.uk/government/publications/maternity-allowance-claim-form
People can fill the MA1 form on their computer or tablet then print it off locally and post it to the DWP.
You can also print a blank form and if needed, have a friend or family member help fill it in with a pen.
If you cannot print a form off the internet, you can order one from DWP.
To do this go to:

BSL users might be able to use the Video Relay Service to order a MA1. You can check if you can use this service on the same webpage.
What if someone needs the MA1 in a format such as braille or large print?
If you need an alternative format to the one available online, you should contact Jobcentre Plus and ask for this.
You or someone who acts on your behalf can call:
0800 055 6688
Or there is a Textphone number:
0800 023 4888
If someone cannot hear or speak on the phone, there is Relay UK:
18001 then 0800 055 6688
What do people need to send back with their claim form?
People need to send in proof of the date their baby is expected such as a letter from their doctor or midwife. Or a MATB1 certificate which they will get from their midwife.
If you are employed and were refused Statutory Maternity Pay, send in the SMP1 your employer sent you explaining why they couldn’t pay it.
You will also need to send in proof of your income such as payslips.
When will I get a decision on my application?
Once you have filled in the claim form and sent this off, you should get a decision within 20 working days.
If your application is successful, we will send you a form confirming your entitlement, how much you have been awarded and the last day of employment (if applicable) before your maternity leave.
Finally, how often is Maternity Allowance paid and for how long?
It is paid every 2 or 4 weeks and payments can start 11 weeks before your baby is due. On your claim form, you will tell us which date within those 11 weeks you want your payments to start.
If you qualify, Maternity Allowance is paid up to 39 weeks.
