• Independent Entrance
• Brick Wall With Plaster
• R.C.C. Ceiling or Roof with Underlying False Ceiling
• Minimum Height 8 feet
• Smooth Floor
Minimum Carpet Area & Storage Facilities
• For Retail Sale (10 Square Meter)
• For Wholesale (10 Square Meter)
• For Retail & Wholesale (15 Square Meter)
• Adequate Storage Facilities Including Cupboards with Glass Doors,Racks & Refrigerator is Required.


Qualification & Documents Required

For License – Allopathic Drugs - Retail Sale
• Forwarding Letter
• Application Form 19 / Form 19 C
• Additional Information Form
• Original Chalan as a Proof of Necessary Fees Paid for the License
• True Copy of Partnership deed or Memorandum of Article of Association & List of Directors, if any
• Employment Letter of a Registered Pharmacist
• Consent Letter of a Registered Pharmacist with Details of Previous Service, if any
• True Copy of Registration Certification issued by Gujarat State Pharmacy Council
• True Copy of Renewal Receipt of Gujarat State Pharmacy Council as a Proof of Registration of the Pharmacist has been Renewed
• Passport Size Photographs of the Pharmacists
• An Affidavit of the Pharmacist
• True Copy of Rent Receipt for the Premises & Necessary Proof for the Legal Possession of the Premises like Copy of the Registered Sales deed etc
• Rough Sketch of the Premises
• Copy of Ration Card or PAN Card or Identity Card of all Partners or Directors & Pharmacist
• Any other Documents that Licensing Authority thinks is Necessary before Granting the License
