Thrombotic thrombocytopenic purpura is a rare blood disorder that causes tiny blood clots to form throughout your body. This can cause the tiny red or purple spots known as petechiae, as well as unexplained purplish bruising that can look like a rash. The bruising is known as purpura. Purpura, also called blood spots or skin hemorrhages, refers to purple-colored spots that are most recognizable on the skin. The spots may also appear on organs or mucous membranes, including the membranes on the inside of the mouth.
In purpura, there are Petechiae is a minute extravasations of blood from very small blood vessels. Petechiae are small 1-3 mm red, non blanching macular lesions caused by intradermal capillary bleeding Purpura are larger, typically raised lesions resulting from bleeding within the skin Classification of purpura;
Nonthrombocytopenic ; means that you have normal platelet levels in your blood.Henoch-Schönlein purpura.
Thrombocytopenic means that you have a lower than normal platelet count. Idiopathic thrombocytopenic purpura or immune thrombocytopenic purpura ITP Thrombotic thrombocytopenic purpura TTPHenoch-Schönlein purpura HSP is a condition that involves swelling inflammationof small blood vessels. The swollen blood vessels leak into the skin, joints, intestines, and kidneys. HSP is seen most often in children between ages 2 and 6. It occurs more often in boys. CAUSES hSP is an autoimmune disorder. This is when the body's immune system attacks the body's own cells and organs. With HSP, this immune response may be caused by an upper respiratory tract infection. Other immune triggers may include an allergic reaction, medicine, injury, or being out in cold weather. Symptoms Rash purpura. Reddish-purple spots that look like bruises develop on the buttocks, legs and feet. The rash can also appear on the arms, face and trunk and may be worse in areas of pressure, such as the sock line and waistline.
Swollen, sore joints arthritis People with Henoch-Schonlein purpura often have pain and swelling around the joints mainly in the knees and ankles. Joint pain sometimes precedes the classical rash by one or two weeks. These symptoms subside when the disease clears and leave no lasting damage.
Digestive tract symptoms. Many children with Henoch-Schonlein purpura develop belly pain, nausea, vomiting and bloody stools. These symptoms sometimes occur before the rash appears.
Kidney involvement. Henoch-Schonlein purpura can also affect the kidneys. In most cases, this shows up as protein or blood in the urine, which you may not even know is there unless you have a urine test done. Usually this goes away once the illness passes, but some people develop persistent kidney disease. Treatment of HSP Laboratory investigation :
Full blood count: Leukocytosis, normal platelet count occasionally eosinophilia,.
Serum complement : C3 and C4 will be in normal level.Immunofluorescence test :Ig A will be elevated.
ANCA and cryoglobulin can be tested but mostly for differential diagnosis.
Urinalysis: proteinuria 2 g/day and hematuria microscopicif there is involvement of kidney

Skin biopsy, renal biopsy and instrumental investigation CT and MRI can be done to check the progression fo the disease. Diagnosis of HSP is mainly based on clinical signs and symptom
Although there is no specific treatment for HSP, you can use over-the-counter pain medicines, such as acetaminophen or nonsteroidal anti-inflammatory drugs NSAIDs such as ibuprofen and naproxen for joint pain. In some cases, corticosteroid medication may be used
