As mentioned in the video, make sure you enter all 3 codes, and loot all 3 locations shown. They give you the upgrade based on the order you loot them. For example, if you looted the first location last, it would end up giving you "Nightsworn Sight IV", instead of the intended "Nightsworn Sight II". Additionally, this took me a while as I spent forever trying to find the third upgrade as I didn't realise it just doesn't show in the feed.
