One thing they ask in aircraft crash investigations is, "Was this aircraft recently serviced?" Here's an example of a PSRU and governor just back from service that fails during takeoff. At about 32:00 you can hear the engine revs increase and see the plane sink as the prop goes to fine pitch, causing decreased thrust. Fortunately the pilot had enough thrust to clear obstacles and circle around the pattern to land again uneventfully.
