⚡️DO I NEED AN X-RAY AFTER AN ANKLE INJURY: Ottawa Ankle Rules⚡️
⁉️How many times have you rolled your ankle and wondered whether or not its a bad sprain versus a fracture ⁉️
⁉️How many times have you waited in the ER for 10 hours only to find out there is no fracture ⁉️
📚Ottawa Ankle Rules (OAR) is a clinical prediction rule to help determine whether or not you require an x-ray based on 5 criteria. If any one of these are positive, you need to get an x-ray. It helps limit the overuse of often unnecessary ankle x-rays in the Emergency Department.
🔑Note: The OAR is effective at determining if you need an x-ray, not if there is, in fact, a fracture present at the foot/ankle. Thus, it is a SCREENING test meant to RULE OUT an ankle fracture not RULE IN one.
🤓Why is this important? If it is a minor sprain, then early weight bearing is encouraged. However, if there is a fracture, you need to spend a specific period of time (depending on the doctors orders based on the type of fracture) offloading (via AirCast boot) or immobilizing (via a cast) for a period of time to allow the bone to heal. If not, complications may arise such as: Non-union, mal-union, foot deformities and longer healing times.

📋Ottawa Ankle Rules: Criteria
1️⃣Inability to weight bear through the leg (If you can limp 4 steps, you are cleared to the next criteria).
2️⃣Tenderness on the navicular bone of the foot (inner mid foot area)
3️⃣Tenderness on the posterior edge of the Medial Malleolus (of the Tibia) and 6 cm up the shin bone.
4️⃣Tenderness on the base of the 5th Metatarsal foot bone (outer mid foot area).
5️⃣Tenderness on the posterior edge of the Lateral Malleolus (of the Fibula) and 6 cm up the fibula bone.
✅If any of these 5 are positive, you NEED to get an ankle x-ray!
